SIGNING UP AND BOOKING

When you sign up with us for use of our services, all we ask
is that you read and abide to our terms and conditions. We also require you to
sign a consent form/ PAR-Q when you first sign up for yourself or child when
using our services for our insurance protection.


BEHAVIOUR & EFFORT

We do not tolerate poor behaviour, theft, criminal damage or
any form of abuse (physical or verbal) towards players, coaches or our
equipment. The coach(es) present of the session has the right to forfeit a
session at any given time within reason, as well as terminating the player from
our services. We also want all of our clients to receive the best help/coaching
we can offer.


We will always prioritise
the player's development. Therefore, if the player does not put commitment into
training, we have the right to forfeit any future training. 


HEALTH

Any health/medical issues that you or your child may have
must be declared, so our coaches are aware of the situation and can enforce
medical action that may be required. We ask you the player (over 16) or the
parent/guardian to bring any medication that you or your child requires on the
site/time of the training session given. Our coaches are also first aid
trained, therefore any medical injuries or health problems you or your child
occurs, the coach on site will do their given best to treat the player and
ambulance services will be called for if the case is serious enough as well to
inform you of the situation if you are not present of the occurrence. If the
player fails to bring any medication that is required, we have the right to
cancel the session, and the session will still be charged.


RESPONSIBILTY/SAFEGUARDING

We will NOT be responsible for any injuries that you or your
child inhabits nor any loss of personal items during any training we support or provide. We will only
declare ourselves responsible for keeping you or your child safe and away from
any harm as well as under full protection from any dangers deemed necessary.


ACCOUNTS

When you create an account with us, you must provide us
information that is accurate, complete, and current at all times. Failure to do
so constitutes a breach of the Terms, which may result in immediate termination
of your account on our Service.

You are responsible for safeguarding the password that you
use to access the Service and for any activities or actions under your
password, whether your password is with our Service or a third-party service.

You agree not to disclose your password to any third party.
You must notify us immediately upon becoming aware of any breach of security or
unauthorised use of your account.


OPTING OUT, CANCELLATIONS & CHARGES

You may opt out yourself or your child from our services at
any given time as long as we are aware. You will still be charged if you or
your child does not make the session that is planned within the 48 hours notice
period without informing us beforehand. Failure to arrive at a planned session
without notice or fair reason will result to termination from our services. Cancellation
due to very poor weather conditions (e.g. thunderstorms, extreme heavy rain,
dangerous winds), unavailability or sickness by the coach, you will not be
charged for the session, and a refund will be given if you have already paid
depending on the circumstances, however, always follow the coach's instructions
regarding this. However, if you feel sick or unwell 48 hours or more prior to
the session, then please let us know to avoid being charged after the 48-hour
notice period. You will still be charged if you cancel the session for any
reason after the 48 hours notice period. If you have any concerns or require
any more information, then please do not hesitate to ask us


INTELLECTUAL PROPERTY

The Service and its original content, features and
functionality are and will remain the exclusive property of Pro-Gram and its
licensors..


PUBLICITY

Achieve 360 produces a range of communications resources to
share and promote our work online through our website and social media. We like
to share the experiences of real people who are using our services in our
communications as it helps to demonstrate the difference our work is making. Therefore,
we offer an optional publicity consent form to be signed if you wish yourself
or your child to be publicly shown on platforms such as our YouTube channel,
social media platforms and other media resources. Please note we do not share
personal details except a name and age although you or your child can always
remain anonymous.


PAYMENTS

All payments must be made when booking a session and before
the training commences. If payment is not received, then the training will not
go as planned. This is normally handled via bank transfer


ATTENDANCE (DROP OFF/PICK UP)

Please note if the child is younger than the age of 16, then
it is the parent/guardian or carer's responsibility to drop off the child and
pick up the child. We prefer you not to leave the premises during the session
period if your child is younger than 16 years of age, also if you are going to
be late dropping off or picking up your child then please contact us so we are
aware.

Training Information, such as the level and positions you play, time and duration of training and training location;

Transaction Information, such as the services/program you purchase or offer for sale through our Services. We may also collect and store limited payment information from you, such as bank account number, sort code, bank name.

Information About Others, such as photos/videos of users receiving our services and the names, email addresses and other information about individuals you invite to use our Services, register to receiving coaching services or authorise to receive notifications; and

Other Information You Choose to Provide, such as when you participate in a survey, contest, promotion, request technical or customer support, communicate with us via third-party social media services or otherwise communicate with us.

What personal data we hold on you:

Personal data means any information about an individual from which that individual can be identified. We collect, use, store and transfer some personal data of our participants and their parents or guardians, and other clients. You provide information about yourself when you book a session, and by filling in forms at an event or online, or by corresponding with us by phone, e-mail or otherwise. The information you give us may include your name, date of birth, address, e-mail address, phone number, gender, and the contact details of a third party in the case of an emergency. We may also ask for relevant health information, which is classed as special category personal data, for the purposes of your health, wellbeing, welfare and safeguarding. Where we hold this data it will be with the explicit consent of the participant or, if applicable, the participant’s parent or guardian. Where we need to collect personal data to fulfill our responsibilities and you do not provide that data, we may not be able to honor or administer your booking. Why we need your personal data We will only use personal data for any purpose for which it has been specifically provided. The reason we need participants’ and clients’ personal data is to be able to run the business and to arrange sessions; to administer and provide the services you are signing up to when you start with us. Our lawful basis for processing your personal data is that we have a contractual obligation to you as a participant or client to provide the services you are registering for.


We have set out below, in a table format, a description of all the ways we plan to use your personal data, and which of the legal bases we rely on to do so. ADVERTISING AND ANALYTICS SERVICES PROVIDED BY OTHERS ​

We may allow others to serve advertisements on our behalf and provide analytics services. Who we share your personal data with? ​


When you sign up for our services your information, will be kept for our personal records. Your personal data is stored by a personal laptop in which the Founder of Achieve 360 has access to. This data may be shared with the company's employees for session information such as planning and point of contact. Consent forms are put in place to make sure that you the player or parent/guardian for children under the consented age of 16 agree to our terms and conditions, by singing the consent forms to accept you agree for us to collect your data such as your or the player's name, age, address, medical conditions/special needs and that you also agree to permitted training sessions with us Achieve 360.


We may share your personal data with selected third parties, suppliers and sub-contractors such as coaches/experts/professionals or session organisers. Third-party service providers will only process your personal data for specified purposes and in accordance with our instructions. We may disclose your personal information to third parties to comply with a legal obligation; or to protect the rights, property, or safety of our participants, clients or affiliates, or others. Our data processing may require your personal data to be transferred outside of the UK. Where we transfer your personal data overseas it is with sufficient appropriate safeguards in place to ensure the security of that personal data. Protection of your personal data We have put in place appropriate security measures to prevent your personal data from being accidentally lost, used or accessed in an unauthorised way, altered or disclosed.

We keep personal data on our participants and clients while they continue to be a participant or client or are otherwise actively involved with us.


Your rights regarding your personal data:


As a data subject you may have the right at any time to request access to, rectification or erasure of your personal data; to restrict or object to certain kinds of processing of your personal data, including direct marketing; to the portability of your personal data and to complain to the UK’s data protection supervisory authority, the Information Commissioner’s Office about the processing of your personal data. As a data subject, you are not obliged to share your personal data with us. If you choose not to share your personal data with us we may not be able to register or administer your membership. We may update this Privacy Notice from time to time and will inform you of any changes in how we handle your personal data.
